Summary: The Medication Management Registered Nurse (RN) is a dual
role that provides direct patient care and is responsible for
medication management and overseeing our Vaccines for Children
(VFC) program. The RN will play a vital role in ensuing emphasis on
patient education, preventive care, and a strong commitment to
improving the health outcomes of our community.Essential Job
Duties: Patient Care: Conduct comprehensive assessments of
patients' health conditions, including taking vital signs,
reviewing medical history, and identifying any existing or
potential health issues. Administer medications, treatments, and
vaccinations as prescribed by physicians, and monitor patients'
progress throughout their healthcare journey. Develop and implement
individualized care plans based on patients' needs, treatment
goals, and medical conditions. Ensure that care plans are regularly
updated to reflect any changes in patients' health status. Perform
various medical procedures, such as wound care, blood draws, IV
therapy, and specimen collection, following established protocols
and maintaining strict adherence to safety and infection control
measures. Collaborate with physicians, nurse practitioners,
physician assistants, and other healthcare professionals to create
a multidisciplinary approach to patient care. Communicate
effectively to ensure coordinated and comprehensive treatment
plans. Maintain accurate and up-to-date patient records,
documenting all care provided, assessments, treatments, and
communications. Comply with clinic policies and legal regulations
regarding patient privacy and record-keeping. Performs independent
face-to-face or telephone triage using written triage protocols.
Performs diagnostic and laboratory tests including but not limited
to EKG, urine dips, urine HCG, etc. Supervises and oversees quality
assurance of these testing services performed in primary care
clinic. Performs patient care including but not limited to birth
control visits, blood pressure checks, medication refills, etc.
Assists with coordination of specialty clinics, such as pediatrics,
dermatology, telemedicine, etc. Assists with meeting the needs of
patients with complex medical problems, including facilitating
referrals and follow-up care as needed. Explains procedures and
treatments to patient to gain cooperation, understanding, and allay
apprehension. Responds to life saving situations based upon nursing
standards, policies, procedures, and protocol. Provides clinical
and administrative support and functions as a resource and mentor
for new staff and students assigned to their work site. Medication
Management Oversee the clinic's medication management processes,
including the safe administration, storage, and documentation of
medications. Educate patients and caregivers on medication usage,
potential side effects, and the importance of adherence.Monitor and
manage medication inventory, ensuring proper stock levels and
expiration date tracking.Implement and maintain protocols for
medication safety and regulatory compliance. Vaccines for Children
(VFC) Program Coordination Manage the VFC program, ensuring
compliance with federal and state guidelines and
regulations.Coordinate vaccine orders, inventory, and storage,
maintaining accurate records of vaccine usage and patient
immunization status. Educate staff, patients, and parents on the
importance of vaccinations and the VFC program. Conduct outreach
and education efforts to increase vaccination rates among eligible
children. Quality Assurance and Compliance Participate in the
development and implementation of clinic policies and procedures
related to patient care, medication management, and immunizations.
Ensure compliance with healthcare regulations, accreditation
standards, and clinic policies. Engage in continuous quality
improvement activities, including audits, data collection, and
reporting. Be a positive role model for all health center
staff.Other duties as assigned.Supervisory

agency staff. Must develop and maintain positive relationships with

written materials, and interpret non-verbal cues during counseling
sessions.Speech and communication: should possess clear and
effective verbal communication skills to facilitate open dialogue
with clients, colleagues, and other healthcare
professionals.Sitting or standing: should be able to sustain a
seated or standing position for extended periods while meeting with
patients/clients/external visitors, attending meetings, or
performing administrative tasks.Emotional resilience: must have the
emotional stability and resilience to work with
clients/patients/coworkers facing challenging circumstances,
maintaining composure and professionalism during difficult
discussions.Sensory sensitivity: should be sensitive to the needs
of clients/patients/coworkers and exhibit appropriate responses to
their physical and emotional cues, ensuring a supportive and
non-judgmental environment.Requirements: Knowledge, Skills &
Abilities: A strong foundation in medical sciences, anatomy,
physiology, pharmacology, and various medical conditions. This
knowledge is crucial for assessing patients' health status,
interpreting lab results, and providing appropriate
treatments.Proficiency in performing a variety of clinical
procedures, such as administering medications, dressing wounds,
taking vital signs, conducting physical assessments, and assisting
with minor medical procedures.Ability to assess patients
comprehensively, gather relevant health history, conduct physical
examinations, and identify potential health issues or risk
factors.Providing compassionate and competent care to patients,
addressing their needs, and educating them about their conditions,
treatment plans, and preventive measures.You must be able to
communicate clearly and empathetically with patients, their
families, and other healthcare professionals.Being able to analyze
complex situations, make informed decisions, and prioritize patient
care based on the severity of conditions and the available
resources.Maintaining accurate and up-to-date patient records,
documenting care provided, and adhering to patient confidentiality
and privacy regulations.Ability to educate patients and/or families
as to the nature of disease and to provide instruction on proper
care and treatment.Ability to develop trust and effective working
relationships with staff members, individuals, families and
external and internal professionals.Effective leadership
abilities.QualificationsValid driver's license and reliable
transportationGraduate of an accredited school of Nursing.Ohio
Registered Nursing License.Active Basic Life Support (BLS)
certification is required.Nursing experience required,
