          • AHA only (Instructor/Provider and RQI acceptable). Letter of completion not acceptable.
          • *Travelers can renew ACLS at the facility.
          • Required for ER, ER OB, and ER Psych.
          • Required within 30 days of start.
          • Required annually.
          • License Verification required within 30 days of start.
          • Required for RNs, LPN/LVN, APs.
          • NIH required for ER and all Critical Care RNs (ICU, MICU, SICU, Neuro ICU, CVICU, and Critical Care Float)
          • Medication Test/Pharmacology Competency Exam required for Aya only. All RN/LVN candidates must complete prior to start.
          • Unit-Specific Competency Exam Required
          • Dress Code - For candidate to review and initial via DocuSign.
          • The Confidential Fraud Waste Abuse and Compliance Reporting Mechanism Form required for all candidates.
          • Fraud, Waste, and Abuse Training Certificate required prior to start and updated annually.
