Based on problem(s) identified, the physical therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and providers and other medical staff. Oversees and supervises provision of care by physical therapist assistants.Minimum Qualifications:Bachelor's Degree: Physical Therapy (Required)LIC - Licensed in Field - State of OhioState of OhioState of OhioState of OhioSPECIALIZED KNOWLEDGE
