Description:Currently, we are seeking a highly organized and
detail-oriented individual to join our team as a Family Medicine
Physician to provide expert medical care to patients of all ages
with a range of potential illnesses, injuries, or disabilities.
Essential duties include conducting physical examinations on
patients, referring patients to medical specialists or counseling
services, wellness visits, and prescribing patients' medications
based on their condition.
Essential Job Duties:
- Provide comprehensive primary care services to patients of all
ages, including diagnosing and treating acute and chronic
illnesses, managing chronic conditions, and promoting preventive
health.
- Conduct thorough medical assessments, including taking detailed
medical histories, performing physician examinations, ordering, and
interpreting laboratory tests and diagnostic imaging, and making
accurate diagnoses.
- Develop and implement evidence-based treatment plans for
patients, including prescribing medications, providing counseling
on lifestyle modifications, and coordinating referrals to
specialists when needed.
- Collaborate with interdisciplinary teams, including nurses and
nurse practitioners, behavioral health providers, and other
healthcare professionals to ensure comprehensive and coordinated
care for patients.
- Educate patients on health promotion and disease prevention
strategies, including lifestyle modifications, vaccinations,
screenings, and early detection of health issues.
- Maintain accurate and up-to-date electronic records (EMR) and
other documentation according to community health center and
regulatory guidelines, ensuring compliance with quality measures
and reporting requirements.
- Facilitate care coordination, patient communication, inquiries,
and engagement.
- Close patient encounters and manage inbox timely.
- Provide culturally sensitive care to diverse patient
populations, respecting their cultural, religious, and social
beliefs, and addressing health disparities.
- Serve as an advocate for patients, promoting their rights and
assisting them in navigating the healthcare system, including
obtaining insurance coverage and accessing community
resources.
- Participate in quality improvement initiatives, such as
performance improvement projects, peer review, and clinical audits,
to enhance the quality and safety of patient care.
- Adhere to federal, state, and local laws, regulations and
policies including community health center regulations, HIPAA,
OSHA, and other relevant guidelines.
- Participate in community outreach programs, health fairs, and
health education activities to promote public health and wellness
in our service areas.
- Collaborate with colleagues and participate in
interdisciplinary meetings, case conferences, and other team-based
activities to ensure coordinated patient-centered care.
- Provide clinical leadership, mentorship, and supervision to
medical residents, medical students, and other healthcare
professionals as needed.
- Provide urgent and emergent care, including after-hours on-call
coverage, as per community health center policies and
procedures.
- Expected to provide the number of budgeted patient visits
annually as determined each year and adjusted with notice- this is
minimal expectation.

Requirements: Knowledge, Skills & Abilities:
- Knowledge of: Comprehensive knowledge of all available services
provided at Third Street, community resources, interpersonal
communication strategies, and computer software.
- Skill in: Communicating effectively in both written and verbal
form, interviewing techniques, managing multiple priorities,
effective time management practices, de-escalation skills,
problem-solving skills, and managing change and conflict.
- Ability to: Manage time and deal with multiple priorities,
problem solving, exercise sound judgment, define problems, collect
data, establish facts and draw valid conclusions, make decisions,
maintain information in confidential manner, listen well, maintain
records and data, use the computer programs and communicate
